For your first intake appointment you will be required to arrive 30 minutes early to complete all of the necessary paperwork for your session.
Please be sure to bring your insurance card with you.
Payment is required at the beginning of each sessions. I do not accept credit or debit cards.
In all but a few situations, your confidentiality is protected by state law and by rules of my profession. However, it's important that you understand that there are limitations to confidentiality. Here are some of the most common cases in which confidentiality is not protected:
*Any indication of harm to self or
others
*Any indication of child or elderly
abuse
*Court order requiring release of
information
*Gravely disabled or inability to care
for self
My fee is $100 for a 50 min. session
I am contracted with most major insurance companies (see list below). Most likely you will only pay a portion of this fee if you are required to pay a copay with your insurance. However, some clients are required to pay a deductible with their insurance company before any insurance coverage is provided.
I am willing to discuss cash-only options for clients who do not have insurance or who do not wish to use their insurance company benefits. In some cases I will slide my cash payment fee below $80 for those clients who are demonstrating economic difficulty.
